Structured well-being programme uptake

Assesses if the organization provides a structured program addressing holistic well-being (physical, mental, spiritual, social) and can demonstrate significant community uptake. Core activities are recurring, structured sessions with defined curricula and registered cohorts (e.g., 6–12 week courses), distinct from one-off events. This aligns with the Maqasid al-Shariah (objectives of Islamic law), particularly Hifz al-Nafs (preservation of life) and Hifz al-Aql (preservation of intellect), ensuring sustained Aafiyah (holistic well-being) within the community.

Assessment Questions
  1. Does the organization have a formal, documented well-being program framework approved by Trustees?
  2. How does the program address the holistic needs of the community across the physical, mental, spiritual, and social dimensions?
  3. Define your cohort attendance calculation and rules for late joiners/cancellations.
  4. What data quality controls exist (duplicate prevention, sign-in verification)?
  5. Can you provide data demonstrating sustained community uptake over at least 2 cohorts or 3 months?
  6. What safeguarding measures (Safer recruitment, LADO procedures, online safeguards) are in place?
  7. What is your lawful basis (Art.6) and special category condition (Art.9) for processing health data, and do you have an APD?
  8. How do you ensure accessibility (audits, gender-sensitive options) and reasonable adjustments?
  9. What facilitator qualifications and supervision arrangements exist for each modality (physical, mental, spiritual)?
  10. What referral/signposting protocols exist, including crisis pathways and information sharing?
  11. What validated outcome tools are used (baseline/endline) and how are results reviewed by the board?
  12. Do you track retention, repeat attendance, and attrition bias?
  13. How do you manage online delivery privacy and safety (Children's Code compliance)?
Evidence Requirements
  • Official documentation of the well-being program framework approved by Trustees.
  • Programme SOP pack: Scope-of-practice, escalation flowchart, crisis scripts, referral templates.
  • Attendance records and cohort calculation logs demonstrating sustained uptake.
  • Data protection pack: RoPA entry, Appropriate Policy Document (APD), retention schedule, DPIA sign-off.
  • Safeguarding records: Single Central Record (SCR) or equivalent, safer recruitment files, DSL role description.
  • First aid needs assessment and provision logs.
  • Quarterly dashboard showing uptake, retention, outcomes, safeguarding incidents, and EDI breakdown.
  • Role descriptions, qualification certificates, and supervision logs for facilitators.
  • MoUs/referral protocols with health partners and information sharing agreements.
  • Outcome measurement data (baseline/endline) with attrition analysis.
  • EDI access plan, accessibility audit, and evidence of reasonable adjustments.
  • Communications plan and participation funnel metrics.
