Post-marital/family counselling service available

Evidence Requirements
  • Policy document: Service Scope, Boundaries & Safety (pastoral vs clinical).
  • Intake/triage form including DASH and child safeguarding screening prompts.
  • Anonymised safeguarding referral log (counts, types, timelines).
  • Sample client information sheet explaining the confidentiality model and couples' consent.
  • Anonymised list of counsellor qualifications (BACP/UKCP/AFT) and insurance certificates.
  • Supervision agreement(s) and anonymised supervision attendance logs.
  • DPIA executive summary and data retention schedule.
  • MOU template + completed due diligence checklist for at least one partner.
